Kidney transplantation represents a significant advancement for managing end-stage renal disease (ESRD), offering improved recipient quality of life and financial benefits over sustained dialysis treatments. Key developments have been made in understanding transplantation immunity and enhancing surgical techniques, leading to standard immunosuppressive therapies. Despite these advancements, long-term allograft survival remains a challenge, with over 50% of recipients experiencing function loss after ten years. Chronic antibody-mediated rejection (CAMR) is a major factor in late allograft failure, exacerbated by multiple immunological and non-immunological factors, including high HLA sensitization, under-immunosuppression, and comorbid conditions like hypertension and post-transplant diabetes.

This Research Topic aims to assess and synthesize comprehensive insights related to kidney transplant outcomes, addressing critical gaps in the management of chronic allograft dysfunction. It seeks to explore various aspects such as the efficacy of current treatments for CAMR, the development of novel diagnostic tools, and the effectiveness of personalized medicine approaches in improving long-term transplant survival.
