Pancreatic cancer has a poor prognosis accounting for a 5-year overall survival rate lower than 10%. This proportion could reach about one-third of patients when considering localized disease and surgical resection represents the only hope for cure. However, only a small proportion of patients have resectable or potentially resectable disease at diagnosis, and local or distance relapse occurs after surgery in most cases. A proportion of patients is not eligible for surgery even after a preoperative therapy.

Adjuvant therapy has demonstrated to prolong survival. Aiming at improving disease-free survival and overall survival of these patients, there is an increasing interest in evaluating the activity and efficacy of neoadjuvant and perioperative treatments. Furthermore, it is of utmost importance to find clinical and translational factors able to select patients who may benefit from a preoperative therapy rather than upfront surgical resection.

This Research Topic aims to summarize current treatment strategies for resectable and borderline resectable pancreatic cancer, to discuss rationale for neoadjuvant and perioperative approaches compared to adjuvant therapies, and to up-to-date novel treatment strategies in localized pancreatic cancer.

We hope to report studies with novel drugs, explore novel predictive biomarkers, and discuss medical and surgical controversies regarding this setting. We will discuss different strategies in not resectable localized pancreatic cancer.

We welcome Original Research, Review, Mini Review and Perspective articles on themes including, but not limited to:
• Neoadjuvant and perioperative systemic treatment in resectable and borderline resectable pancreatic cancer.
• The role of genomic and transcriptomic profiling and other molecular biomarkers in the localized setting
• Surgical approaches and controversies in local approaches in pancreatic cancer
• The role of old and novel locoregional therapies
• Adjuvant strategies after surgical resection and emerging predictive factors

Please note: manuscripts consisting solely of bioinformatics or computational analysis of public genomic or transcriptomic databases which are not accompanied by validation (independent cohort or biological validation in vitro or in vivo) are out of scope for this section and will not be accepted as part of this Research Topic.
