Neuropathic pain (NP) afflicts as many as 7% of the population. It contains peripheral NP (pNP) and central NP(cNP). Clinically, pNP is more common, and with the aging of the people, its incidence rate increases yearly. Simple drug therapy and non-interventional therapy can only provide temporary or partial relief. Long-term management of chronic pain is an urgent problem. Surgery has proved to be effective for several pNPs. However, many aspects are still waiting for discussion, including indications, opportunities, surgical schemes, etc. pNP is not a single disease, but a syndrome secondary to different diseases and lesions. It can be broadly divided into two categories, neurotumorous and non-neurotumorous, such as trauma and chronic injuries which include diabetic neuropathy, viral infections like postherpetic neuralgia, etc. A considerable part of pNP is the primary or secondary peripheral nerve tumor (PNT) manifestation. The occurrence of pNP after PNT surgery has also been reported. Atypical but disturbing clinical manifestations and their fragile structure make diagnosing and treating the disease a significant challenge. Therefore, the best operation plan, intraoperative precautions, avoidance, and treatment of postoperative pNP are also the interests of this topic. Peripheral nerve injury is still a common trauma disease that may cause chronic pNP and is very difficult to treat. In recent years, significant progress has been made in surgical treatment and basic peripheral nerve injury research, such as exploring new surgery methods, making new peripheral nerve injury models, promoting the injured nerve function recovery, some nerve substitutes, and even the changes in brain function.
The purpose of this study was to provide the latest information on surgical management of pNP and to discuss and share the clinical experience. And we aim to promote the early diagnosis, optimum treatment, and better prognosis of PNT, peripheral nerve injury, and other diseases that cause chronic pNP through the discussion of the critical point of diagnosis and treatment, surgical treatment plan, precautions during operation, and the pain complications.
