Sarcoidosis is a chronic inflammatory disorder characterized by the formation of non-caseating granuloma in target organs. Although it is generally considered a benign disease, sarcoidosis is associated with significantly higher mortality and morbidity rate when compared with healthy controls. The heterogeneous clinical course, challenging differential diagnosis, and the varying responses to treatment through individual subjects, make a personalized approach in the clinical management of patients with sarcoidosis absolutely necessary. Despite recent advances, the comprehension of pathogenetic processes of sarcoidosis is far to be fully clarified, especially concerning the biomolecular mechanisms underlying the progression of disease and different phenotypes (including sarcoid localizations).
The main goal of this Research Topic is to provide a spotlight on the biomolecular pathways and immunological features of sarcoidosis through a “bench to bedside” translational approach. The new technologies, including -omics sciences and next-generations cytofluorimetric analysis performed on different biological fluids (serum, blood, tissue and bronchoalveolar lavage), are expected to provide a “great leap forward” in the comprehension of pathobiology of sarcoidosis and, consequently, in the discovery of new biomarkers to be implemented in the clinical management, especially concerning differential diagnosis, prognostic estimation, severity assessment and prediction to response to treatment.
