In the last decades, there has been pressure to guarantee the sustainability of healthcare systems despite increasing resource constraints, which is leading to progressive reorganizations of priorities. The focus is on boosting quality for patients while increasing cost-effectiveness. Within this mindset, “Value-Based healthcare”, which is the maximization of patient health outcome per dollar spent, has implications for the provision of health services and the related financing mechanisms. In order to align all stakeholders of the health system, Value-Based concepts have extended to include other dimensions of care for the well-being of those working in health systems (Berwick’s triple aim) and population health as a whole (Bodenheimer’s quadruple aim). Ever since, efforts to bring this approach to care to real-life settings have called for reflections on the societal values of interventions (i.e. environmental health, evaluation of innovative treatments and technology) and to applicative challenges such as the identification of the most appropriate and comprehensive outcome indicators, for which ulterior research is needed.
The objective of this Research Topic is to contribute to the existing body of knowledge in this field with a clear picture of the current scenario of the Value-Based approach in oncologic clinical practice, to deliver sound experience on its impact, potential benefits, challenges to address and future research needs and public health.
In particular, at this stage, we presume that the central topics are as follows:
a. Patient centeredness and shared decision making
b. Patient centered outcome monitoring
c. Application of personalized care to clinical pathways
d. ‘Green’ oncology
e. Organizational and financing implications from reshaping the delivery of healthcare services according to a Value-Based care approach
Among the determinants for the change, the following stand out: refinement of performance indicators to include patient perspective (PROMS/PREMS); reshaping of reimbursement systems to bundled payments, based on clinical outcomes; implementation of shared decision-making as routine in clinical practice; reshaping of logistics and operations to respect the values of a ‘green’ care delivery; digital support for the implementation of VB approaches.
This Research Topic welcomes submissions around the implementation of Value-Based oncological healthcare (across care settings, specialties and specific health issues), as well as the impact of the application of VB healthcare in oncology.
