Colorectal cancer (CRC) is a leading cause of global mortality as the third most common cancer with approximately over 1 million patients affected annually. CRC patients have a low overall survival rate and poor prognosis primarily due to the difficulty and challenges with early detection. This leads to CRC progressing to an aggressive state before it is able to be diagnosed. Early detection of CRC patients typically have a 5 year survival rate of 90% which unfortunately, significantly decreases as the disease progresses to a later stage.
There have been recent developments in early detection of CRC to help improve the prognosis including the screening of adenomatous polyps, fecal occult blood testing, sigmoidoscopy and optical colonoscopy, which is the most widely used CRC screening technique. However, some of these diagnostic techniques are invasive and therefore, not all CRC patients go through the screening processes which decreases their survival rate. There are alternative minimal invasive screening methods such focal and blood tests, however these are dependent on the changes in the tumor microenvironment to suggest the development of CRC. Therefore, these methods are not as reliable for early detection.
There have been recent studies including immunotherapy, a developing field in oncology as potential diagnostic markers and therapeutic targets dependent on the molecular mechanisms of the disease. There have been recent studies involving monoclonal antibodies which target epidermal growth factor receptor (EGFR) including cetuximab for the treatment of metastatic CRC patients and this has led to a consistent improvement in the survival rate of patients. Other studies have explored how T cells are affected in CRC patients and in particular, the role of CD8+ T lymphocytes, which are known to have antitumor properties. Other diagnostic tools include fecal immunochemical test (FIT), DNA methylation and assessment of bacteria relative levels, which are non-invasive screening tools for CRC detection and early screening.
