Oral progestins have been widely utilized to inhibit ovulation in the past in the development of hormonal contraception. Recently, oral progestins have been also suggested as an efficient method of preventing premature LH surges during ovarian stimulation in IVF, with the aid of vitrification processes.
Progestin-primed ovarian stimulation (PPOS) offers an attractive alternative in terms of similar pregnancy outcomes compared with GnRH analogue in clinical trials, however, the current available evidence about PPOS is limited, and its exact role in IVF should be evaluated in future studies. As a stimulation protocol mandatory to freeze-all, PPOS is a good alternative protocol for women who do not contemplate a fresh embryo transfer, such as fertility preservation, oocyte donors, and preimplantation genetic testing. In addition, PPOS has demonstrated advantages for controlling LH levels in refractory cases such as extremely low ovarian reserve. Novel clinical research from different regions and countries are necessary to demonstrate its efficacy, safety, and potential scope of application.
This Research Topic will collect recent original research and reviews covering advances in PPOS in basic and clinical research. We are especially interested in the following themes in this Research Topic:
· The application of PPOS in some specific populations such as polycystic ovarian syndrome and extremely low ovarian reserve.
· The comparison of PPOS and GnRH analogue in ovarian stimulation (such as original gonadotropin dose, trigger timing and dose, oocyte potential and embryo competence).
· The cost-effectiveness of PPOS in combination with freeze-all
· The mechanism of LH suppression by progesterone during ovarian stimulation
