Heart failure (HF) represents a significant cause of morbidity and mortality in childhood and adults affected by congenital heart disease (CHD). Etiology and pathogenesis are different between adults and children, in children, it is most often due to structural heart disease and reversible conditions. Early diagnosis and improvements in surgical and interventional techniques, post-operative care, and medical management in recent years have resulted in unprecedented survival of patients with CHD. Despite remarkable success, patients often develop cardiac complications, including HF.
HF is a complex syndrome with heterogeneous etiology and presentation. In the clinical setting, ensuring an accurate diagnosis and defining etiology is essential to optimal treatment, a better knowledge of HF may be significant to understand better and treat physio pathologically and develop therapeutic strategies. In the end-stage disease, heart transplantation is the best choice of treatment. At the same time, a left ventricular assist device can be used as a bridge to transplantation (due to the difficulties in finding organ donors), recovery (in the case of myocarditis), or destination therapy (for patients with systemic disease).
In this Research Topic, we will present and discuss different aspects of HF of children and adults with CHD, including etiology, physiopathology, specific treatments, pulmonary hypertension, heart transplant in pediatric age, and patients affected by CHD in end-stage of HF. We will bring together a group of investigators broadly across the field who have evaluated and applied physiological aspects to advance and improve their care, from basic to clinical research. Original Research Articles and Reviews are encouraged.
