Intracranial atherosclerotic disease (ICAD) is an important cause of ischemic stroke worldwide. Patients with ischemic stroke attributed to ICAD face a considerable risk of stroke recurrence, despite following guideline-recommended treatment strategies. In recent years, advanced imaging techniques have emerged to portrait different aspects of ICAD and ICAD-related ischemic stroke and novel treatment methods are under investigation. It is hoped that as a result, more accurate risk stratification and more effective secondary stroke prevention of patients with symptomatic ICAD will be developed. However, research on ICAD still falls far behind that on atherosclerotic disease in other vascular beds, such as carotid and coronary arteries. Numerous questions remain unanswered in this field, e.g., reasons underlying the ethnic difference in ICAD prevalence, evolution of the disease under treatment, differences in risk of stroke relapse and response to treatment in ICAD-related strokes of different mechanisms, differences in hyperacute endovascular treatment of intracranial large vessel occlusion due to ICAD and other etiologies, potential benefit of stenting therapy in selected patients, and safety and efficacy of novel treatment methods in ICAD, etc.
In this research topic, we therefore solicit high-quality, review or original research articles that can help update/advance our knowledge on ICAD and ICAD-related stroke.

• Epidemiology of ICAD and ICAD-related stroke, the difference between populations or evolution over time
• Risk factors and mechanisms of ICAD development and progression
• Mechanisms of ICAD-related stroke

• Prognostic factors of stroke occurrence and recurrence in ICAD
• Conventional and novel imaging in ICAD and ICAD-related stroke, and the clinical implications
• Conventional and novel treatment strategies in symptomatic ICAD
• Prognosis of ICAD and ICAD-related stroke
