Zinner syndrome (ZS) is a rare congenital urologic disorder characterized by a classic triad of manifestations: ipsilateral seminal vesicle cyst, ipsilateral ejaculatory duct obstruction, and ipsilateral renal hypoplasia. We present a case of a young male patient diagnosed with ZS and treated at our center. Imaging revealed absence of the left kidney, along with enlarged, dysmorphic, and abnormally dilated seminal vesicle glands, as well as multiple tortuous tubular extensions of the seminal vesicles into the bladder. Cystoscopy confirmed the presence of a cystic mass in the bladder with elongated seminal vesicles. Semen analysis, biochemical indices, and tumor markers showed no significant abnormalities. In this post, we present a case of ZS with abdominal pain as the primary symptom. The patient underwent laparoscopic left nephroureterectomy and recovered well postoperatively, with regular follow-up provided thereafter. We discuss its pathogenesis, diagnosis, and treatment, and provide a literature review of the disease.
