FRAIL Scale Predicts Early Postoperative Shoulder Function and Long-term Satisfaction in Patients Undergoing Arthroscopic Rotator Cuff Repair: A Prospective Case-Control Study

Objective: This study aimed to investigate the relevance of the FRAIL Scale as a predictor of early postoperative shoulder function and long-term satisfaction in patients undergoing arthroscopic rotator cuff repair (ARCR).Methods: A total of 80 patients who underwent ARCR and postoperative rehabilitation training in our hospital from November 2022 to September 2023 were selected as the research subjects. The modified 5-item Frailty Index (mFI-5) was used to assess the frailty status of all patients at admission, and the patients were divided into non-frail and frail groups. To assess postoperative shoulder function, the Oxford Shoulder Score (OSS) and Visual Analog Scale (VAS) for pain were administered to patients at 1-, 6-, and 12-months post-surgery. The ROC curve and Logistic regression were used to assess the predictive value of the frailty scale for early postoperative functional outcomes and patient satisfaction.Results: A total of 80 patients met the inclusion criteria. The frailty assessment results were negatively correlated with postoperative function (r=-0.271, P=0.015). The ROC curve analysis results showed that the area under the ROC curve (AUC) of the frailty scale for predicting postoperative complications, medical complications, surgical-site infections, hospital admission, and adverse discharge were 0.676, 0.642, 0.679, 0.718, and 0.739, respectively. The preoperative frailty score was inversely correlated with the postoperative satisfaction score (r=-0.577, P<0.001), and had the ability to predict longterm patient satisfaction (AUC=0.863, p<0.001).Preoperative frailty level is associated with early postoperative functional outcomes and patient satisfaction after ARCR.
