Abstract: Cardiovascular disease poses a significant challenge to global health, and this review is dedicated to exploring the role of exercise as a non-pharmacological intervention in this disease category. Specifically, our focus is on Irisin, a myokine released during exercise with superior potential for modulating metabolism. Irisin has demonstrated remarkable efficacy in metabolic and neurodegenerative diseases. The objective of this paper is to unveil its distinctive contribution in the cardiovascular domain. We offer a comprehensive summary of Irisin's therapeutic capabilities and corresponding mechanisms in cardiovascular diseases (e.g., atherosclerosis, myocardial infarction, myocardial ischemia-reperfusion, heart failure) and cerebrovascular diseases (including ischemic stroke, hemorrhagic stroke, and post-stroke depression). Additionally, we delve into the relationship between Irisin and perivascular adipose tissue. Through a thorough discussion spanning molecular mechanisms to clinical applications, this paper elucidates the intricate network of Irisin's regulatory role in cardiovascular health. Our work not only opens new avenues for future clinical applications but also underscores the unique significance of exercise and Irisin in cardiovascular health.
