Factors affecting posaconazole plasma concentrations: A metaanalysis and systematic review

This manuscript investigates the factors that influence the concentration of different posaconazole formulations. We systematically searched PubMed, EMBASE, and the Cochrane Library to identify studies assessing factors that influence blood posaconazole concentrations. We included 46 studies involving 8,505 patients. Co-administration of drugs affecting posaconazole metabolism significantly reduces its concentrations. High-fat nutrition, age, and sex did not significantly influence posaconazole oral suspension (POS) concentrations. Diarrhea markedly reduced concentrations in both delayed-release tablets (DRT) and POS. Neither vomiting nor mucositis significantly affected POS concentrations. Acid-suppressing agents such as H2 receptor antagonists and proton pump inhibitors markedly decreased POS concentrations but did not affect DRT. Comparative studies of dosage forms demonstrated significantly higher concentrations in DRT than in POS. This study discusses changes in posaconazole concentrations in a variety of patients and dosing conditions. I am confident that this manuscript meets the high standards of scientific rigor and originality expected by the Frontiers in Pharmacology.
