A Rare Case of CNS-CLL Mimicking Demyelinating Disease: Multidisciplinary Approaches for Accurate Diagnosis

    Central nervous system involvement by chronic lymphocytic leukemia (CNS-CLL) is an exceptionally rare and diagnostically challenging condition due to its nonspecific symptoms and lack of definitive diagnostic tests. We report a case of a 58-year-old male who was initially diagnosed with autoimmune-related demyelinating disease, presenting with neurological symptoms suggestive of demyelinating lesions. Despite treatment, the patient's condition worsened, leading to further evaluation. A comprehensive multidisciplinary approach, including detailed neurological, hematological, radiological, and pathological assessments, ultimately led to an accurate diagnosis of CNS-CLL. Subsequent targeted therapy resulted in significant clinical improvement. This case underscores the critical importance of a thorough multidisciplinary diagnostic process in identifying and managing CNS-CLL, which can mimic other neurological disorders.
