A case we display of elderly female with congenital dextrocardia. Due to severe palpitations, electrocardiogram revealed paroxysmal atrial fibrillation, medical history unveiled cerebral infarction and renal failure. Firstly, in the treatment of paroxysmal atrial fibrillation, anticoagulant is required in our treatment planning. NOAC drugs are recommended in guidelines. However, NOAC is metabolized by renal function that cause poor efficacy in patient undergoing dialysis. In addition, warfarin may cause fluctuations in INR, which is not conducive to controlling symptoms of cerebral infarction. In order to improve the patient's life quality, we focus on the unique challenges in one -stop procedure that occur from dextrocardia.
